Some things I would like to know:

1. Dates of your project inclusive of pre-production and wrap.
2. Type of Project e.g. Music Video, Short, Feature etc.
3. Position: Gaffer/LD/DP etc.

Some things you should know:

1. As I am not a rental house, I do provide gear as part of the service and convenience. The gear does not go out without me being hired on the project.   
2. I do not bid on projects.  
3. I absolutely do not work for: “DEFERRED” or “COPY, CREDIT, MEAL” as terms of payment.
